  • Fixed Codex harness failure handling so missing validation dependencies, shims, runners, tools, or binaries are treated as validation-tooling gaps instead of task failures.

  • Updated follow-up prompts so validation-tooling gaps do not require `Failure:` reports while real failures still require `Failure:` and `Error details:`.
